NYC Care clients receive a personal membership card which allows them to access health care at any NYC Health + Hospitals location across NYC, a list which includes 11 hospitals and dozens of community-based health centers. Clients are also able to choose their own primary care provider and get support from a full primary care team. NYC Care clients are able to access many different medical services, including: • Primary and Preventive Care • Specialty Care • Behavior Health and Substance Abuse • Women’s Health • Medications • Vision

To qualify for NYC Care you must meet the following requirements: • You must live in the five boroughs of New York City. • Must not qualify for any health insurance plan available in New York State • Not be able to afford health insurance based on government guidelines. • Anyone can apply regardless of immigration status. ​ Those who only have Emergency Medicaid are eligible to apply for NYC Care.

Anyone can apply for NYC Care regardless of immigration status. The application status does not ask for social security number of any immigration documents. Applying for NYC Care will not affect your current and/or future immigration status.

In order to enroll, you will need three types of documents: • Identification (Passport, ID NYC, NY State ID, Foreign ID, etc.) • Proof of address (Utility Bill: Con Edison- Light Bill, Letter from the mail, Lease, etc.) • Proof of income (Pay stubs (last 4 weeks), Employment Letter, Support letter, etc.) Proof of citizenship or immigration status is not required to enroll. If you are unable to provide the required documents, a counselor may still be able to help you enroll. We encourage you to call even if you aren’t sure you’re eligible.
